Like PvP, it is really the character that is facing challenges. The Environment is controlled by the Storytellers, so this is Storyteller controlled challenges. This is often called "Plot" for short.
The most obvious source of conflict is Feeding. Vampires are always hungry and nobody wants to be prey. Every vampire has a Predator Type chosen in character creation and a Hunting Pool based on it. This can speed up the process to one chop. Alternatively it can go blow by blow. This is a narrative storytelling choice. And storytelling choices are made by, you guessed it, Storytellers.
They enforce the Masquerade better than any Prince, by hunting and killing every vampire they can find. Hiding from them is a necessity, but many vampires also work to discredit them and make their murderous work more difficult. Risking attracting their notice is obviously dangerous.
Anarchs are actively contesting for control of the cities, and player characters are likely to be on both sides of the conflict. The Sabbat does not, but may just diablerize or otherwise murder vampires its packs come across. Independents don't follow the rules and could be dangerous.
Vampires like to think they are the secret apex predators of the world. Are they, though?
